INSERTION ORDER
 The advertiser/agency must prepare an Insertion Order for each advertisement to include the following:
• Insertion Order Number
 • Name of Advertiser
 • Billing Address
 • Date(s) of Insertion
 • Section or Position Request, if necessary
 • Size and Cost of the advertisement
 • Description of the Advertisement (i.e., headline)
 • Contact Name and Phone Number
